By the longest dimension of the design, in inches or centimetres — a “3-inch tattoo” is 3 inches across at its widest point regardless of shape. Artists quote and price from this measurement plus the level of detail inside it.
How does the true-size calibration work?

Screens differ in pixels-per-centimetre, so the tool shows an on-screen outline of a bank card — the one object with identical dimensions worldwide — and you drag until it matches a real card held to the screen. From then on, sizes shown are physically accurate on your display, and the setting is remembered.
Why can’t I get a detailed design really small?

Ink spreads slightly in skin over the years. Fine lines packed closely together merge, so a detailed design shrunk small turns into a smudge — which is why artists push back on “can you make it smaller?”. Simplify the design to shrink it, or size it up to keep the detail.
What size should a lettering tattoo be?

Keep script at least about 2 cm tall, more for fine or high-contrast faces. Thin letterforms are the first thing that blurs as ink spreads, and legibility is the entire point of a word.
